Making a complaint

First, always contact the company and business from which you purchased the good or service by phone, e‑mail or mail. Submit the complaint as soon as possible after noticing the defect.

You can submit a complaint by calling the company's customer service line, filling out a form on the company's website, or sending a free-form e‑mail or letter.

Complaint templates assist with submitting a complaint or feedback to a company
Complete the form and save it to your computer, or print the form before filling it out. Send the completed form to the company as an e‑mail attachment or through the regular mail. Make a copy of the complaint for yourself.

If the company has a customer service form on its website, you can copy the information from the template into it.
